PROGRAM STATUS

Exclusive to individuals transitioning out of homelessness; limited funds available.

WHAT DO I NEED?

We will require applicants to come to the FACC at 514 S. Chicago Ave. to fill out an application. Applicants must provide a photo ID and proof of income. Listed below are the available types of assistance and their specific requirements:

  • If applicants have a lease agreement, they will be required to provide their lease with their landlord’s name and contact information. It is required that the applicant’s name is on the lease in order to receive assistance.

  • Applicants must provide their bill. Bills must be in the applicant’s name in order to receive assistance.

  • Applicants must provide all relevant documents and information. Contact Us for more specific requirements based on your request.

Our program only serves applicants that are transitioning out of homelessness and operates on limited funds which means that we may not be able to provide financial assistance to everyone who applies.

Please note that submitting an application does not guarantee that you will receive financial assistance. Our team will review each application carefully and make decisions based on available funds and the specific circumstances of each applicant. We will do our best to assist as many individuals as possible, but we may not be able to fulfill every request.
